Other Names: Hazelton Cottage (1911), Ulmer Cottage (1928), Clark Cottage (1933); DIS

Year built: between 1897 and 1900

Other information: Built as a single-family residence, a number of cure porches have been added over several years; one encircles the west and south sides, topped by a two-bay sleeping porch on the south side and a third-floor. The northern facade has a three-bay cure porch on the second floor above the main entrance. There are three apartments.

The 1917 health survey indicates no TB patients in residence, but by 1925, the house was registered as a private sanatorium, operated by Miss Marie Stuckman as a boarding cottage with care, with three rooms for rent on the second floor, and two on the third floor, all with cure porches; rates ranged from $23 to $30. In 1928, a Ms. Ulmer ran the house, with a fourth room added, with no porch. The T.B. Society inspection noted that Ms. Ulmer was a "good practical nurse." The 1933 records list Mrs. L. Clark as the operator.
