Description | The Pediatric HIV/AIDS Program (PHAP) at Children’s Hospital & Research Center Oakland provides comprehensive state-of-the-art medical care and treatment for children and youth (newborn through 21 years) with known or suspected HIV infection, AIDS and AIDS related symptoms. Established in 1986, care is provided through Children’s Department of Infectious Disease. |

Support and Social Services Our social workers are an integral part of our multidisciplinary team and usually meet with all patients and families during clinic visits to provide support and psychosocial education. Between clinic visits, social workers often have phone contact with patients and/or their families to follow up and provide support and case management services. Group support and recreational activities are also offered throughout the year for patients and their families. Social workers are generally available to help address and discuss the following:

Social Activities Most summers, our program is able to offer a few recreational outings to help build community among our patients and families. In December, we celebrate the holidays with a festive party. All patients seen during the year and family members are invited to this annual event, which includes food and entertainment.
Transitional Support Between the ages of 18 and 21, patients typically leave our program to “graduate” to a provider or clinic that serves older adolescents or adults. After years of seeing the same doctor, nurse and social worker at Children’s Hospital, it can be difficult for some teens and families to make this big transition. To help make the process go as smoothly as possible, our team starts addressing transition issues well ahead of time, usually when a patient is between the ages of 12 and 14. Our staff members seek to provide any needed support related to healthcare transition, including addressing insurance issues, identifying potential adult providers, and facilitating introductions between the patient and adult clinic staff. |
