Services

Support and Social Services

Our social workers are an integral part of our multidisciplinary team and usually meet with all patients and families during clinic visits to provide support and psychosocial education. Between clinic visits, social workers often have phone contact with patients and/or their families to follow up and provide support and case management services. Group support and recreational activities are also offered throughout the year for patients and their families.

Social workers are generally available to help address and discuss the following:

  • Housing and financial concerns
  • Health coverage and insurance issues
  • Transportation needs
  • Relationship and disclosure support
  • School and educational needs
  • Employment

Families are urged to contact their social worker to discuss their social service needs.

Social Activities

Most summers, our program is able to offer a few recreational outings to help build community among our patients and families. In December, we celebrate the holidays with a festive party. All patients seen during the year and family members are invited to this annual event, which includes food and entertainment.

Transitional Support

Between the ages of 18 and 21, patients typically leave our program to “graduate” to a provider or clinic that serves older adolescents or adults. After years of seeing the same doctor, nurse and social worker at Children’s Hospital, it can be difficult for some teens and families to make this big transition.

To help make the process go as smoothly as possible, our team starts addressing transition issues well ahead of time, usually when a patient is between the ages of 12 and 14. Our staff members seek to provide any needed support related to healthcare transition, including addressing insurance issues, identifying potential adult providers, and facilitating introductions between the patient and adult clinic staff.

Clinic:

What to Bring: Parents/Guardians

Please come to your clinic appointment with:

  1. Your child’s immunization records
  2. The name, address and telephone number of your child’s primary care physician
  3. Insurance information/cards
  4. A list of all medicines taken by your child, including non-prescription medicines and vitamins. If you don’t have a list, please bring the containers.

Clinic Policies

  • Child supervision: Children under the age of 12 must not be left without adult supervision in any area of the hospital. This policy helps ensure the safety of young children and protects the clinic from being held liable for any injuries sustained by unattended children.

  • Brothers and sisters are welcome at Children’s: Please do not bring brothers and sisters if they have colds or other contagious illnesses, as we do not want other patients exposed. Please make sure your children are supervised while in clinic. Please bring books, a snack or something to hold the attention of siblings so that your attention can be focused on the child in clinic.

  • Visitors: Please do not bring relatives or friends into the clinic room who do not know your child’s diagnosis. If you are unable to avoid bringing someone who does not know, ask them to remain in the waiting area.

  • Missed appointment: Missed appointments compromise our ability to provide quality care. Please cancel appointments that cannot be kept. This should be done as far in advance as possible. Early cancellations give other patients the opportunity to utilize available appointments. Appointments can be cancelled by calling Joyce at 510-428-3337, your nurse or your social worker.

  • Late appointment arrivals: In order to give you and our other patients the highest level care possible, if you are more than 15 minutes late, it may be necessary to reschedule your appointments, and you may incur a rescheduling fee.

  • Failure to follow-up: It is our hope and expectation that patients and their families follow treatment recommendations. If you and your family disagree with any treatment recommendations or have questions about them, please notify us immediately so we can fully discuss these issues. Sometimes, when treatment recommendations are not followed over a period of time, our team may become concerned about medical neglect. In this case, we would be obligated by law to contact Child Protective Services.
